AstraZeneca’s Camizestrant Combination Receives FDA Accelerated Approval

AstraZeneca recently announced that the FDA granted accelerated approval to camizestrant (Etcamah) in combination with a CDK4/6 inhibitor for adults with HR-positive/HER2-negative locally advanced or metastatic breast cancer. The indication applies when an ESR1 mutation is detected during first-line aromatase inhibitor plus CDK4/6 inhibitor therapy using an FDA-authorized test.

Approval Highlights

  • SERENA-6 reduced the risk of progression or death by 56%
  • Median PFS: 16.0 vs 9.2 months
  • Guardant360 CDx approved for ESR1 mutation detection
  • First FDA approval to guide a treatment switch before radiographic progression using ctDNA

The significance goes beyond adding another treatment option. This approval links a treatment switch to a molecular signal rather than waiting for disease progression to become visible on imaging. Precision oncology is increasingly moving from deciding which treatment fits which patient to also defining when treatment should change.

Because this is an accelerated approval, the FDA has required confirmatory studies to verify clinical benefit, including whether intervening when an ESR1 mutation emerges before confirmed disease progression ultimately translates into meaningful benefit for patients.

Congratulations to AstraZeneca, Guardant Health, and everyone involved in this approval.
